Enhance display of module loaded in batch

FA lacks the ability to show __all__ modules loaded into a batch address space. Currently FA focusses at the call chain and the modules registered by LE at the abending enclave. The mini dump does not contain the relevant control blocks to identify all modules loaded via STEPLIB/JOBLIB and LNKLST: CDE/LLE chain from RB.
During complicated problem analysis a display of all modules in the Job Pack Area together with their entry point address, link date and indication from which loadlib they were loaded.

    This functionality can be achieved by using a Formatting user exit to buzz the CDEs and calling EVALUATE for the desired length to ensure the relevant storage pages are included in the Fault Entry. I have written a basic user formatting exec to do this which I am happy to pass on to you for your evaluation/use.
