Prevent users from debugging excluded programs

Due to many reasons (privacy, performance, ...) organizations might want some programs to be excluded from debugging. Currently, the command NAMES EXCLUDE allows for that. However, it lacks fine-grain control over that exclusion. Worse than that, it can be easily bypassed if some deferred breakpoint is in place.

This IDEA proposes to implement a more flexible yet robust exclusion mechanism supporting the following features,

1. Disallow to set up breakpoints for excluded programs.

2. Weather breakpoints take precedence over NAMES EXCLUDE should be configurable. 

2. Users/groups who can/cannot debug excluded programs should be configurable.
