Include program object offset in dump data for inuse heap storage

In a TPF Dump the ECB inuse heap nodes display a header with the module name and module offset of the GET or REL programs, whilst the heap trace shows object file name and object file offset.
This enhancement is to request that the inuse heap node header also includes the object file name and object file offset rather than module offset.
