Improve z/TPFDF dump handling if a device is not operational

When a device is not operational (i.e. IDECSUD is set to CXSGNO/X'88' after a FIND macro) any of the following could occur:
1. Streaming OPR-DB0100 dumps might be issued as every access on that device will fail. In this situation these dumps are not helpful and should be suppressed.
2. A DBCRE command (create prime file address) might hit a FIND error and no core block is set up on the DECB but the error is not checked and no error indicator is set for the caller. Any subsequent access command with that SW00SR will then run into an OPR-DB010E dump when trying to ATTAC a core block that was not detached. The error should be checked so that subsequent processing will generate the expected DB0100 system error. The same error check is missing in a similar process in the segment that handles DBCKP.
