Add field names to the streamed CSV when using Generic HTTP subscriber.

The Generic HTTP subscriber is a great option to be able to integrate with 3rd party and opensource tools.

The CSV format is easy to exploit and adapt.

However it has no header and only contains the raw data, which greatly reduces its usefulness.

The only way to get the field names is to go in the documentation and manually add them as you receive the data, or to use the Kafka protocol instead of the Generic HTTP one, and the Kafka protocol is really not that easy to integrate (especially if you do not have Kafka in your environment).

The idea is to add the field name as a header line to the CSV output, this would increase a lot the usefulness of the Generic HTTP protocol.
