Provide ability to provide subsystem ID for any application and roll remaining IMS/DB2 special ISPF PDB panels into base

In the SA ISPF PDB dialog:
Currently, there are only a few (IMS and DB2) applications that provide a field to specify an MVS subsystem ID.    This along with a few remaining definitions require some unique coding a panels.

Recommendation:
Provide an MVS subsystem ID field on the Application Information panel.
Provide a sysplex ID field in the same panel (or move to Messages User    Data panel)    to be used for such things as identifying IMSPLEX and DB2 Data Sharing names.
Move remaining DB2 definitions into Messages/User Data panel
