Remove the requirement for the IGZIUXB (IGZIUXC/IGZIUXD) exit with the COBOL v6.2 TEST(SEPARATE) option implementation

Without implementing the the IGZIUXB exit (or it's CICS version IGZIUXC/IGZIUXD), CEEDUMP processing is unable to access SEPARATE DWARF debugging data in the COBOL v6.2 implementation. This results in the local variables and line numbers not being displayed, greatly reducing the usability of the dump.

The exit was optional previously (ie. in the COBOL v4.2 and earlier implementation of SEPARATE debug data), so it's not clear why it is required now.
